He failed the trial because he attacked "Vader" in anger. (Look at Luke's grimace before he takes off Vader's head.) Remember, Luke thinks Vader killed his father.
It is hate not fear that makes him fail the test. He did not run away and fought with anger not peace.
Later Luke tells Yoda,"I am not afraid (to face Vader)" and Yoda reminds him "Remember your failure at the cave..." Yoda is implying that like Luke is susceptible to giving into his "hate" and following the dark side of the force.
For we are not at war with flesh and blood but with powers and principalities. When you try to destroy the villain rather than the evil he personifies, you end up missing the mark and taking on the very evil that you are at war with. In the end redemption destroyed the evil within Vader.
It's supposed to imply that Luke's recklessness and headstrong nature will lead him to become just as bad as Vader. It's more foreshadowing than anything, since Luke hasn't decided to save his friends yet.
@Pedros1and2 the forest is filled with enteties of the force (from Jedi and sith) whome ever enters the cave then the enteties become of vision of that persons worst fear. Luke had a vision of vader but when he (WRECKED) vader it was revealed that He's afraid of becoming a sith
@Pedros1and2 It shows if you attempt to defeat an enemy by fighting it you become it yourself. The only way to truley defeat the enemy is to not fight at all.

I know why unaltered. It's the original theatrical version of Episode V- TESB from 1980 not the remastered editions on VHS in 1997 or DVD in 2004.
