Ghost=dead. Even if he survived Shepherd he'd die by makarov. He'd shoot a general over one man, so what makes you think he'd be BFFL with Makarov? Makarov killed nearly one-hundred thousand people in the first two games! Not counting the "ww3"(yeah not really) Ghost died before that though. Shot may have pierced his arteries, etc. Plus, point blank with a .44/.50 (shepherd likely uses a modified one) would've at LEAST caused bone shattering. The location of the bullet may have pierced his lungs

Fire RESISTANT (Nothing is fire proof, in fact, the suit likely wasn't meant to handle extended heat) suits wouldn't do crap against a gasoline fire. Not only would the HEAT from the fire cause serious medical issues, but the prolonged burning of gasoline might've worn away the suit. Even if he did survive the fire, a .44 or .50 cal (shepard likely uses a modified pistol) bullet to the shoulder would be enough to do a ton of damage.
