Im not sure myself, but what ive heard, is this.

Some guy was creating a chemical to allow humans to breath underwater, or to make them stronger (?) and Adam was unstable and it made people mutate and go mental. Then a Neural gas escaped and flooded the city. This is when it all fell apart and people want psycho and I guess then the place was destroyed?

actualy its pretty easy to miss, you only notice it because a turret shoots at you from the inside

Bioshock is semi-free roam. Visiting the dentist office isn't required :P so you can avoid scares like that
