When they reach almost no health they will kneel and enemies will cease to attack them. Followers will only die if further attacked by the player in this kneeling low health state, either by normal attacks or area of effect damage.

Both major wikis currently (December 16) note that kneeling followers will regenerate and normally not be targetted by enemies, but that a large enough amount of damage from dot effects or enemy AoE can still kill them. The exceptions are followers with true Essential flags.

This will be solved for sure once the Creation Kit is out.
