I'm sorry to disagree with you. NASA did see the piece of foam insulation come off but thought that it was low enough in density that it probably couldn't do any damage. The Shuttle was also not equipted on that trip for an extra vehicular walk to check it. They couldn't send someone out to check it and if it did do any damage, they knew it couldn't be fixed anyway. They just took a chance. Nothing else to do.
The tech is there. We can and should make something science specific. The shuttles were cold war tools first and foremost and IMO they are retired because they are not needed. Science was always an afterthought and the friggin wiki and politics are 100% proof of this.
As for reliability,it is 100%. There were NO shuttle failures. There WERE bad management decisions that countermanded safety engineers requests and actions. 2 of them.
the american space programs is a classic, Bite more than you can Chew...our technology is still too primitive to accept the challenge of space...we need at least another 2 to 3 hundred years to feel confident about getting men back and forth from space in as safe as going to the grocery store.

I told my brother in law that i had a weird dream . i was in a neighborhood with single family homes it was night and i was standing outside,all of the sudden the space shuttle appeared it had these beautiful colors on the bottom and it they seemed very peaceful as it kept going it finally was out of my view and then i heard an explosion i said to my self it blew up then i woke up.2 weeks later in happened. i feel sad :(

Not true, they use many different formulations of foam based on location. They have had foam strikes in the past with varying amounts of damage. This one was particularly unusual in that a 2 foot section fell off and hit right on the wing leading edge. A glancing strike wouldn't have caused nearly as much damage.

No one knew it was there, so no one knew there would be an issue. NASA could have had the crew take a look at the wing just to make sure, but the people who studied the lift off video thought that there would be no issue. They were wrong. I wouldn't go blaming environmentalists. It was a mistake by NASA.
