I used Sony Vegas. Its made up of a few color adjustment effects, a blurred copy of the video and alpha gradient PNG for the depth of field effect. And of course sped up.
In Vegas, i had two tracks of the clip on the timeline (both sped up and color corrected) and blurred the bottom track. Then I created a black and white gradient PNG that was matched up to the clip so the white area of the PNG would be clear and the black area would be blurry. I applied the PNG as an alpha mask to the clear clip track so the blurred track would show thru. As mentioned in other comments, its framerate is too high and needs to be cut down.
@gimlitheone Imo it works best at 4 FPS. 24/25 FPS is just too much and takes away alot of the miniature effect. This would have ben 10 times as good if he shortened the amount of fps :)

I think it would help a lot to take some frames of, so it's seams that it was done with a photo camera. just a thought, good vid.

Is it possible to do the reverse, in other words, to film a miniature and to turn into a 'real' image?

@puttings I agree on that one. As we can see, result is already very good. Less frames would give even better result.
