You say the backhand is the most difficult shot is the topspin backhand - which is understandable, but perhaps part of the problem is how far round the grip is? Even in regular tennis I can't think of a single player that is that far round for a single handed backhand. But this is just my opinion and I can't say I know much about wheelchair tennis :).
You don't necessarily have to turn out away from the court on a backhand. Take a look at the top players in the World. They will turn in on a backhand. It depends on the shot.
