I always assumed true-form-Saya had a more greenish, almost Cthulhu-esque form, compared to the fleshy, skin-colored messes that Fuminori perceived people as, and not just because of her otherworldly heritage and the general green-motif of the game. Other than the green slime trails she'd leave behind, I always assumed her hair and eye color were a kind of "red-right-hand" as to her true appearance.

The protagonist (Fuminori) is in a car crash that kills the rest of his family, and he survives thanks to experimental brain surgery. Unfortunately, he now sees the world as though it was hosed down in gore and people look like disgusting meat monsters. All except Saya, who looks entirely normal.
@shimazu32 I think I get it now, Human Saya is what the main charecter sees because of his mental distortion but in reality shes really a hidious creature. watching the playthrough still but I'm starting to understand what she is.
