Actually the missile was fired at McCain when he was within striking distance of his bombing target. He could have avoided the SAM but instead chose to continue toward his target. As his bombs were dropped the SAM struck his right wing and he plummetted to the ground. The choice he made showed McCain's extraordinary courage.
Although McCain himself jokes about it, claiming "I was such a good piolot I could even intercept Sruface-to-air missiles" Not just a hero, but a modest one.
