Actually, the OMG face is actually a surprised face which usually shows when you get hit from behind where "he" isn't expecting it.

That face is ment to appear when you take more then 20 damage, but the programmers messed up and it appear if you gain more then 20 HP when you take damage

spartan, the doom guy goes like " :O " whenever you ge hurt, but while being hurt you get health - say an im shoots me while im on top of a medkit and i grab the medkit while the screen is red - doom guy will make that ridiculous face :P

That´s called "ouch face", which appears when you recieve a large amount of damage, or like in this case, when you recieve damage and in the same time you pick up a large health bonus or you do IDDQD cheat. It really is a bug of the program, and solved in posterior versions.

You get the face from taking heavy damage.For instance shooting a barrel next to you.

You can legitimately get that Doom face in I believe episode 3 of the first Doom. There's a room with lava on the ground and an invulnerability sphere. When you're running on the lava into the sphere you'll see the face.
