it isnt. Watch and read your manga. In a cover for one chapter, Kubo draws ogichi and has written down besides the hollow in kanji OGICHI, Ichigo written backwards. (since japanese does not use HCI in any form of writing, He use CHI.) That is where everyone came up with that name. And Shirosaki is given by people back when he first appeared in Bleach, when he and Ichigo battled over control of zangetsu during Zaraki and Ichigo's fight. Shiro was the opposite of black, fan name. So Shirosaki.

Since Ichigo's dad is a shinigami Ichigo was born with shinigami powers: he was born with Zangetsu inside himself. Zangetsu tried to tell Ichigo his name, but Ichigo couldn't hear it. Rukia just awakened the powers and gave him more powers too.
