I lived in China for almost 3 years... it IS like that, even in cities. If you go to a mall, there will usually be doors on the 'sea level toilets' but you'll NEVER see toilet paper. you have to bring your own. even fancy restaurants sell you tissue paper (in a pouch) for about 2 RMB - about 30 cents - to use in their washrooms. I've been to places way outside the city where there wasn't even a pony wall between the toilets - you could pat the guy on the shoulder who's crapping beside you...

25 years ago, in Spain, I had to pee. I went into a small closet, and urinated into a hole in the floor. I could not have imagined having to take a shite in there. My goodness, it was a small pub, but the whole place was filthy. It shocked me. Third world? What happened to the second world. Even England was much, much cleaner.
If you think about it it may be more sanitary...your ass never actually touching a toilet seat that someone definitely pissed on before you. I've been to china a few times and I never saw any bathrooms that were any dirtier that an old texaco or shell station bathroom here in the states.

@Jarik1996 Although that is true what you said, i have recently just come back from backpacking around China and South East Asia and i found this clip to be quite an accurate interpretation of the majority of the places in China i visited. However i still really enjoyed my time there and would definitely recommend it to other people. The only downside i found to the country was the food but i'm a fussy eater anyway.
