The S-IC/S-II staging is from a Saturn V, but the S-IVB separation is from a Saturn IB, not a Saturn V. Every S-IVB used on a Saturn V had two solid-fuel ullage rockets, not the 3 shown here.
The IB flight was probably AS-203 because one of the 3 S-IVB ullage rockets can be seen to not perform quite as well as the other two though it was still in spec.
