Very nicely drawn, but something about that body positure doesn't seem very House-ish. (Since caricatures most often portray the most quintessential of a character.)

He's using layers. If you look at 0:45, he colors it, then at :053 he switches layers to work on the grayscale, then he combines the layers at 2:05 so it looks like the color just appears.
