Dr. Peng are you able yo perform TKA with this bi-block alone. Do you ever have trouble from possible pain related to obturator nerve? If so , how do you deal whith it ?
Thank you for your comments. As you said, I had to cover Tourniquet pain and deep pain by hammering the femoral bone, which can't be blocked by femoral and sciatic nerve block. In my practice for more than 100 cases of unilateral TKA anesthestized by this bi-block, I routinely used midazolam 5~7.5mg and fentanyl 50~100ug to cover tourniquet pain, and propofol 30mg bolus each time(totally around 100mg) before hamming the femur . You can also use propofol infusion (20~30ml/hr) also.
