In realism mode your teammates don't glow through walls so you have to stick together, so if a hunter or charger pounces one of your teammates and you aren't nearby they pretty much have no hope. Another thing that separates it from campaign is that zombies don't die as easily so you have to shoot them in the head.

Not exactly, it replaces the healthpack, so you have to have your friends heal you, which uses up a healthpack, or use pills and adrenaline, which is only a temporary boost, so i think the defibrilator is a balanced, strategical item.
