I don't really get what you mean but, here I will break it up to you.

You will be given the choice to make a deal with Dimitri or kill him.

If you kill Dimitri, your girlfriend Kate is killed by Pegorino in a drive-by at Romans wedding. You still get the money at the end of the game.

If you made the deal with Dimitri, you get the money but your cousin Roman is killed by a hitman of Dimitri at his own wedding..................
