It's got the same base design as mine, somewhere around that time they redesigned the base to have more rounded corners. The water level in the tank looks like its too high, the flapper on the 3.5 gpf version should close right at the conclusion of the flush.
This Gerber Model is similiar to one of the Briggs models and one of the American Standard models

I very strongly suggest you buy a flip ultra camcorder. Very good quality and great easy to use. Strongly suggested.

You're right. When I looked for the date stamp, I noticed that the water level was about two inches above the fill line.
