The number to the left is your current skill level, the number on the right that says "requires" is what the skill needs to be before you can get that perk. So for instance Restoration requires 20, but your resto is currently 18. So you'd need to increase it by 2 before you can spend a perk there.

Your skills increase as you use them, so to increase Restoration 2 more levels you'll just need to use Healing more.

You need a higher level in your skills to unlock higher level perks for them. I suggest you spend that perk on your two-handed skill, since you seem to prefer huge ass axes and swords. Btw, the two-handed hammer is the most powerful type of weapon in the game.
