Properly fitted and with a TIGHT chin strap. Too many people ride with loose straps because tightening it is "uncomfortable". I teach remedial math and had a brain-damaged student whose bike helmet slipped back when he was hit by a car and 3 pieces of skull ended up on the ground. Coma for 9 days, near death, and came out badly disabled.

The rider's chin strap should be as tight as the horse chin strap! Put a finger or two underneath, open the jaw enough to "take a treat", but no more.
