@somehominid That probably means that you joined an sv_pure 1 server. Servers with this setting disable custom skins and models of any kind. If you want to be able to use skins like this bear, you need to join servers that have sv_pure 0.

you're just switching the models so yes it works anywhere, other people wont see it though unless they have the mod too

so basically if you install it, any heavies you see will be bears

For some reason the claws and fish only appear in the "created" server for me, but won't appear in public servers, any ideas?

Custom skins and models only work on servers with sv_pure 0 enabled. Which means that the server you play on isn't able to show your custom stuff.
