Penny dies because Dr. Horrible has worked so hard for what he wants (to join the Evil League of Evil) that he neglected what he needs (to get the courage to ask Penny out) and because of this he lose the other thing he wanted. Penny. This is partly because he thinks that he can use what he wants (the League) instead of what he needs.
