  • This is not to insult the guy; I want to avoid these type of injuries. Any trainer will tell you that in weight lifting "proper form is very important"; his flaw was his form. For those who don't know; the rule of thumb for barbell squats is to stand at the width of your shoulders. If you stand wider than your shoulders, you could get injured and if you stand with the heels closer, you could lose your balance. Oh, and remember that in squats, the knee becomes very vulnerable so use proper form.
