@judge348 It's because his Visored Hollowfication is not a part of his Zanpakuto, but rather a part of his overall power. Unlike the Espada, which get all of the mask features when they release their Zanpakuto. In Ichigo's case, it's simply and extend of his power. And add-on, if you wish.

The last three have released forms. Number 3 has water based attacks and had a fat sword. Number two turns into something like the grim reaper/death, number one doesn't change much, he get an eye patch from lilllionette, and he get two guns that fire cero, and he can have Lillionettes spirit and his turn into wolves. And then Yammies release form is Number 0.

Ulquiorras second release form has never been seen by any of the other espada or Aizen.
