  • I heard that the engineer mismarked the brace position that should go directly under the outriggers on the floor below, and they cought him remarking them in chalk in the correct position immediately after the incident. Tisk tisk.

  • This is the one at Newcastle Australia? The engineers had propped the lower floors and as far as I knew, they marked the top floor with X's for the outriggers. This crane couldn't be dismantled because of it's size (300t I believe) so they had to demolish the car park around it and go from there. It was one of the safety topics in my advanced riggers course.

  • looks to me like they didn't take into account the load put on the outrigger with the crane being scoped that far out with the weight on just one foot. you got to take the load moment into consideration or you'll put yourself in a bind when workin on soft ground or a floor with empty space beneath you.

  • this is the reason you have floor analysis done and you always use spreader plates the operator , owner, and engineers should have know better

  • There are soooo many things done wrong here. Apparently this was the first job for this brand spanking new LTM1250 set up with full stack and the kidney's to boot. I can't remember exactly the weight but I think it's around 212,000 lbs of ctwt alone! Shoring wouldn't have done squat here. That much weight concentrated in one spot on a lousy parkade? And this was not only the operator at fault, no crane this size gets dispatched with one phone call. Remarkably stupid.

  • it was not the crane operators fault, it was the engineers... they signed off on the crane to be set up there and they did have props, but not enough for the load there were carrying :)

  • BULL. The operator is 100% to blame for setting that up over a room or crawl space. If they said with proper shoring it would be ok the operator should have checked it before setting up. 100% OPERATOR F-UP

  • a crane that size should never be on a slab unless it was adequately propped below! no steel mats either.the operator needs a fucking with a sledge.i,d have liked to have seen the method statement for that job!

  • yes pads not down big mistake. i drive a demag ac200-1 from my exp concrete is the most misleading surface a crane can set up on you never know whats under it. and as for the comment on the safety officer in my country the driver has the final say on all such issues, these guys merely advise. hope drivers ok smashing machine.

  • engineer onsite overrode operator about using pads under the outriggers and signed off on the setup. they had already done some panels aswell. slab fault i think.

  • bigjakesrq is right it was setup on a suspended slab that was only 120 thick where the first outrigger punched through and it took 4 hours for it to go right through so it would be a boring vid
