The MACS-Lift has two variations: a)the simple MACS-Lift (S-MACS) in which we place two purse-string sutures and b) the extended MACS-Lift (X-MACS) in which we place an additional-third purse-string suture to suspend the malar fat pad and correct the nasolabial groove, the midface and the lower eyelid.
@splasticsurgery Was the MACS lift shown in the video the 2 stitch variety or the 3 stitch. It looked like a 3 stitch with the undermining over the malar region. I could have seen that wrong. If this is 3 stitch have you ever had a problem with the dog ear at the lateral canthus?
it is extended MACS-Lift using 3 stitches because I want to correct the nasolabial groove and the midface. In these cases of X-MACS Lift is always necessary to perform lower blepharoplasty (or skin excision only from the lower eyelid), in order to correct the skin redundancy which occurred from the skin elevation at a pure vertical vector. Sometimes for better result I combine the MACS lift with sort scar temporal brow lift in order to correct the dog ear in temporal area.
@splasticsurgery I did one of these this AM. I did the lower lid bleph bilaterally but did not have to remove much skin. I was able to work the temporal skin reduction out without doing the brow lift in addition. The 3rd stitch makes all the difference. Raising that fat pad is key with the deeper nasolabial grooves but I pretty much had to do a lower lid bleph like you say.
The temporal brow lift is optional and not necessary in all cases. I perform MACS lift also in elderly people with much skin laxity and brow ptosis. In these cases the temporal lift is good option with satisfactory result (under local)
Maybe I saw it wrong, but with the addition of the third stitch onto the malar fat pad, wouldn't this be an extended macs or X-MACS? Also, you've got a very good assistant.
