Probably because they actually rap about things that are true, rather than something about cars, or spinnahs, or that sort of thing. This song is real, about how, in the ghetto, you likely to die before the ambulance get there. That may explain why such "bad rappers" got air time.

sure, he may be an old washed out fool now, but the rap group public enemy was the first rap group to make the majority of it's songs about political issues. In other words, this is one of the best hip hop groups ever made, period. Ask any fan of artists like Kool G rap, Rakim, KRS, ATCQ, Saigon, Immortal technique, etc., they're probably going to slap you upside the head for even doubting public enemy's influence.
