It means to follow the same routine over and over again whilst getting worse results each time.

"To go round in circles" is to repeat things without ever achieving anything. If the circles are always getting smaller (ever-decreasing) things get worse each time.

It says in an interview with Richard Briers they couldn't think of anything and at the last moment the producer thought of it, because that's how Martin felt, they thought it was a bit long title, but stuck with it
