Since he was even slimmer when he debuted in WCW, it's most like causing him to not produce enough thyroid hormone, making it VERY difficult for him to keep his weight down. He had surgery on his pituitary gland when he was in college to slow the progression of the disease.

The fact that he's come back in much better shape recently shows his dedication.

@fakizer123 Big show had Giantism much like Andre the giant. He had a tumor on his pituitary gland which kept him growing and sped up his metabolism to pretty much inhuman proportions. When the removed the tumor ( and possibly the gland ) his growth stopped and his Metabolism slowed to a comparative crawl. His weight and health ballooned from there.
