If you have even a small understanding of the speech in japanese then you'll know that boys and girls actually have different forms of speech. When Haruhi says Usa Chan is cute she inadvertenly speaks says it with a girls speech. Not her eyes, but not wanting to change in front of the twins is a normal girl thing, but it is possible her speech or even pitch in voice made them figure it out.Mori most likely felt her bra cause I doubt weight would make him blush. Kyouya just knows everything.

It's possible that Mori figured she was a girl becuase of how he responded to her cry for help. We've seen that he's a gentelmen and it wouldn't be that far of a stretch to see it being an immediate reaction to help a lady in need. He doesn't even seem to give it a second thought before he's out of his seat picking her up and it's after that action that he figures out she's a girl. Plus if you listen closely her voice does raise in pitch when she calls to him, maybe another little clue?

The fact Haruhi was a girl was spoiled to me by a story synopsis. It was only until I watched half of the anime that I realized that viewers were actually supposed to find out Haruhi was a girl by themselves.
