In the military it is "SIR" or 'MA'AM' that is the proper way for an enlisted man to address any officer or for an officer to address any superior officer. So the General was responding the way he was taught. Boxer should have accepted that the general was not not showing any sign of disrespect to her.

I haven't seen the whole dialogue, but it looked like there was already tension between the Brigadier General and the Senator (judging from her inquisitory "why has it been delayed?"). The General's "Ma'am" in the start of the sentence was also like "Look!/Dude!/Here's the thing".
