  • @munam87 means 2 positive(ELEVATION) pointing up......IN 2 LEADS .....2 (DEPRESSIONS) pointing down....IN 2 LEADS..... if you dont have "2 up and 2 down" you dont a STEMI going on....

  • It's a great point. I have seen this twice, as acute occlusions presented as a LBBB.

    Because time is critical, I would defer this to the Interventionists immediately.

    This is based on the AHA criteria for STEMI.

  • Interventionists will tell you that treating a LBBB as a STEMI is not good care based on how seldom acute occlusions are found in LBBB. Unless there are clear indications for ischemia why would you fibrinolyse or PCI?
