In the beginning (when Byakuya defeated him en took Rukia with him, but he was safe because of Urahara) the second time that I remember is when Ulquiorra his hand Ichigo's body pierced, but he was safe, thanks to Inoue. The third time is when Uilquiorra used is special zero and pierced right through his body and fell. (He was safe after he was transformd into a Vizard and returned to normal)
