Isn't it obvious? The big sword that slowly floats down towards the square platform does huge damage to anything or anyone still on the platform when it hits. The point of the achievement is to have an abomination standing there with depleted health, so that when the sword lands it kills it. Pretty simple when you know how the boss works.
