The grudge is a ghost because her mother always helped ppl who had scary experiences by taking a bit of blood from the ppl but the mother fed "The grudge" the blood of those ppl and in the grudge 3 the real grudge dies but her sister turns into the new grudge because she had to drink the blood of the grudge without getting killed you can see the part with her mother in Grudge 1 or 2 idk which anymore

@SladeHums the grudge refers to the curse that happens in the movies. When someone is killed by someone who is in a fit of rage, it basically turns them into a ghost/evil spirit that haunts/kills those who come around the place where they died
