Well... If everybody is already guessing about the meaning of this, i shall give it a try myself.

The "Thank you" at the end is absolutly correct. as the dog said "if you would have a tumor, youd have a true friend". that doesnt mean, the tumor is the friend. but as he is deadly ill he truely got a friend: the dog. in this miserable situation the dog was there - and the situation was because of the tumor. so the dog gave advice so he could proof, that hes a real friend - and give his live?

@MrMunchhead Indeed, I believe it does. See, what I take from it is that because the tumor was going to kill him, his dog revealed himself as our protagonist's "true friend." As such, he thanks the tumor at the end for helping it all come to be. Yeah, it's messed up, but that's how I see it.
