Ouch! I think perhaps that playing with the kitty vs try to make the experience calming may have a bit to do with her unwillingness to sit for the injection. Its not play time. Using "doughnut" around the neck would prevent biting.
Kitty restraints are needed. (Thinkin' like a nurse, lulz). Seriously, wrap him up ina blanket or restrain him some other way, kitty ain't gonna get his fluids, clearly he isn't having any of this. Sometimes you have to be mean to be nice.
I have honestly never seen an attempt at SQ fluids so horrible! Get someone there to restrain the kitty! Or make a kitty burrito!

Has it got any easier? My cat was diagnosed with diabetes so I have to give him insulin injections. His reacting to the needle like your cat is here.
