No the game itself does not need to be downloaded. the plugin is used on the internet browser and the data required is much smaller than pretty much any free online first person shooter.

There are servers with different skill ratings and such. I think the ones that support the ranking system are limited to your skill level. Then there are unranked servers. Surprisingly enough this game runs crazy smooth. If you can run the original Quake 3 Arena this shouldn't be a problem.
