After 8 years and retreating their troops, the Iraqi government is under the influence of Iran (majority of its cabinet members are from the Shia party formed by Iranian Ayatollahs in mid 80s, The Supreme Council) and after 10 years Afghanistan is still controlled by Taliban and other warlord factions with no end of violence in sight!

Hostage rescue and special operations don’t fail because you’re the US Special Forces or succeed because you’re the British SAS or the Israeli Commandos. 100% of the mission can be split down to planning 99.9% luck 0.1% but that 0.1% can turn the whole thing in to a disaster.

If the terrorist carrying the grenade in the Iranian embassy had pulled the pin the SAS’s assault would have been an international disaster and embarrassment to the British. If the terrorists at Entebbe had detonated the explosives in the terminal, operation thunderbolt would have been a disaster. If the rescue attempt at Iran had been a success the American Special Forces would have been classed as the world’s best. IMO it depends how that 0.1% falls!
