My uncle used to have the same fan in the roof-mount version. It got replaced several years ago (the motor wore out), but the silver mushroom top from the original fan still remains.
Also, I believe the fan is manufactured by a company called Lomanco, but I'm not 100% sure.
Yes, obviously this is an older Lomanco (the newer version has a quieter motor similar to the attic air you showed us). The roof mushroom for the Lomanco models also has 3 screws in the middle that take on the shape of a triangle.
You install this thing yet for the summer?

I think that fan is availible in the roof mount version too. I strongly recognize the motor.
By the way, does that fan have a start-up noise when it turns on?

this is close mine is a Lomanco but the roof version would look like the attic air but with this motor and ring
