no hoffman is dead even though it does no show him dead he will surley bleed to death, for that mask is a variation of the mask from number one, which if you got back and look at it carefully has 2 large spikes inside the persons mouth which rips it open so when the mask opens slightly with hoffman it would impale fairly deep in his mouth and he will bleed to death considering he is locked in the room

From what I saw he could easily get out. The Bear trap lodged between the bars of the door was powerful enough to burst them open (as shown at 9:09) making for an easy escape through the opening. But still, he has a half ripped open jaw and broken hands.
