When you watch this movie for what it is, it becomes a true masterpiece. People were expecting a scary as hell film, but that's not what Ridley wanted this to be at all.

Try watching it once more, with an open mind. I guarantee you that you'll find it of much higher quality than when you first watched it. I've seen it only twice, yet I see it as possibly the best Lecter movie made.

He is alive because the brain feels no pain. And due to the fact that he knew what and where to cut is the key, he cut it in a spot where it would make you more confused but not kill you, it would kill you over time.

However there is no chance he would ever be able to live a normal life after, because he brain has been damaged and also open to the air and thus to infections.
