Galfond didn't only hate the river, but Antonio wanted to call out it was shit card and when Klein bets at it, i don't think anyone at Galfond's level is able to call that - meaning, he knows he is behind more often than not and a bluff here is almost never in Klein's range given Klein is incapable of bluffing so big on such a draw heavy board.
The reaction is enough confirmation you need to show that was a sick bluff. Unlike most people who are spewy fishes and think 'ZOMG STRAIGHT!!!!!!!!'

I don't think the penalty is really anything more than an excuse for player to go to the bathroom, get air..etc Im sure that to continue to break the rules could become grounds for longer penalties or lead to disqualification. I've never seen it come to that though. This game above is being played for cash so the players make their own rules, im only talking about tournaments btw.
