Nice. That last chord is actually 2 chords. This first is just a Db major 9. So a Db major triad, plus the major 7th, and then the 9th. So the notes are...

Db, F, Ab, C, Eb

Or, just play a Db bass with your left hand, and play a F-minor 7 with your right hand, same thing.

Then he hangs onto the Db bass, and plays an Eb major in the right hand. He bounces back and forth between those 2 chords every 2 beats.
