For starters, I don't think this is acne at all. Based on what I'm seeing, the doctor is using a small laser to make tiny lacerations for malformed sebaceous glands. Most people have proper sebaceous glands that allow for pimples to form in the first place, meaning, that a slight infection has an opening for the pus to go. This person seems to lack the openings, or properly formed sebaceous glands, thus they cannot drain naturally. It may leave some scars, but its better than infection.

A lot of acne is hereditary. I've known people who've had it very bad, and others who've maybe had a couple pimples from 5th grade to sophomore in college. A lot of it has to do with hormones and not hygiene. Leaving that amount of pus under the skin is incredibly unhealthy and if it accumulates too much it can leak into the bloodstream and go to the brain.
