This is indeed Nakamura Kenzo sensei, you can also see other senior Nakamura ryu teachers in the background. Good clip

The guy at the beginning is the late Nakamura Taizaburo sensei's nephew. I had the pleasure of meeting him and several of the other senseis from Yokohama when they visited Washington D.C. earlier this year.

The video is of Nakamura Ryu. Done by the honbu dojo in Yokohoma. I believe that is the Yokohoma Shrine.

I don't know, but it looks to be Toyama-ryu. There is a rather well known Toyama-ryu group in Machida-shi, near Kawasaki.
