To me (at least what i believe):

This song is (just) mostly about 2 mentally-illed persons (Personality Disorder, Mental Disorder, whatever) trying to live a peaceful life, in a place where they aren't allowed to, they're just trying to survive through their disease, be it with each other, or BOTH travelling in their own world, imagining the world in THEIR world.
