Although you can realize this clearly in very early begining, you cann't do it until you really suffer badly from pain of dehaydration. That's why he waited to cut the arm until the last minite. although he knew his arm is dead long time ago. He was so afraid of losing his arm.

The part of the arm he amputated was fine so believe me it would have hurt like nothing you can ever imagine and hopefully never have to go through. But it was his determination to live that kept him awake when he had to do that and it was that determination that kept him going on the 7 mile journey to help after he'd cut off his arm. He's an unbelievable inspiration

Then he realized the blade wasn't sharp enough to cut through his bone, so he had to break his wrist. In two places. Then he began cutting again. And also he didn't cut off his arm on the first or second day of being stuck.. he did it when it was the very last thing to do. I think we never know what we - the human beings- are capable of until we find ourselves in a particular situation. I think I'd probably do the same if I had nothing else to try.. whether I would survive that is a different issue...

In the movie, James Franco who played Ralston, admitted that he was a bit selfish for not telling anyone where he was going, and it's understandable that he didn't. Who doesn't want to get away from the world to get a little bit of fresh air? I'm sure if he knew what the outcome of his actions would have been though, im sure he would have drawn a detailed map of where he went and gave it to everyone he knew -_-
