They do die, but then something happens, like pushing a reset button, which sends them back into a different time stream where they all live happily ever after. I technically don't know if I'm right, but I think thats what happened.

@1824jules incorrect for the most part, he dies of hypothermia after Ushio dies then the Ushio in the parrallel universe sacrifices herself to send Tomoya to a universe were nagisa never died. She can do this because she is part of a higher dimesion and can freely alter the dimensions below her. Almost like how if we minipulated the 4th dimension (time) then we could teleport because we could fold the third dimension like a peice of paper creating a bridge from one side of the paper to the other

Ushio sacrifices herself to give Tomoya a chance to relive his life from the point Nagisa died if he can collect the few small lights around the world that contain Ushio's (Dream World) best memories by doing so he is put into the parallel universe in which Nagisa never died. Ushio can do this because the alternate dream world contained a hidden higher dimension and by bending the third dimension is able to change it

@videogamescoverftw Actually If I'm correct, It was just a dream, but I could be wrong, but either way, I think it was intended that the Nagisa Death arc was just a dream because it's what Tomoya feared the most, so ontop of that, they they put it in so he could understand what his father went through when Tomoya's mother passed away. and since he remembered it, he was even more overjoyed to see it not come true and have happened in that exact way

When Nagisa was at labor the way he describes her was that it's "too painful to watch" and thus he started to think the worst case scenario where Nagisa died. (his fear totally took over him since sanae says that it might be dangerous to let Nagisa have ushio) so the sad part never did existed.
