You just impressed the hell out of me, i didnt think anyone would ever do it. As far as i can tell you are the 1st person to see ending 5 outside myself. Even the other people involved in working on the game never saw it. To get special achievement endings you have to do something extreme for each one. For example, have a ton of money, kill everyone who can be killed, rape everyone who can be raped, infect all possible people, etc.

The 5th ending is very hard to acheive (like the special endings).

The first thing which you need is to get an item from the lifeguard at the beginning. It may seem impossible to get it, but it can be done with alot of luck.

You're at the one puzzle in the game which relies on timing instead of thinking. Pay attention to what the guards are doing. Time your move so you wont be heard.

Is it the one in the room with shelf and the desk? My advice... go back to the room with the nude girl on the wall and pay more attention to her.

Mermaid's foam is one of the harder puzzles. Do you have any sort of semen? If you dont or cant make any then you cant ever get the mermaid foam. If you do try to use some in the right place to lure out a mermaid.

Manipulate the hair, then use the rope on leg irons.  Getting out is easy with a grappling hook.
